This questionnaire is prepared for both your benefit as the prospective owner, and the Bearded Collie that you plan to have become a part of your family. Please answer the following questions, print it out for your records, and then return the form by clicking on the "submit" button at the end of the form. If you are an AOL customer, AOL does not like on-line forms. Please fill it out, copy your answers (control-C), then paste them into an email (control-V), and send to the webmaster at lgdlover@yahoo.com. Be sure to cc: yourself so that you have a copy for your files.Thank you for your cooperation



AOL USERS PLEASE NOTE:
The use of AOL with online forms is a problem; AOL has issues with online forms. They don't seem to mail the form properly through their browser and mail like IE and Netscape do. I have no idea why.
To overcome this as an AOL user, after filling out the form, try highlighting the entire form with your mouse or cursor, and copying the form and answers from the browser you view it with (use "control C"). Then paste it into a word processor or e-mail (use "control V"). Then you can email it to both me (lightnnrod@mac.com) and yourself. This way, you will also have a copy, as well.....webmaster
